| Hate to break it to ya, but...Any crash or mishap caused by a preventable maintenance issue is still pilot error (assuming the pilot is the one doing the maintenance - as is usually the case with RC stuff).
I had the tail rotor belt snap on my Shuttle causing it to pile in. I knew the belt had a bad spot, but thought it would be okay. I thought wrong. So that one went in the Pilot Error column.
The best way to prevent "mechanical failures" causing a crash? Three words: Preflight, Preflight and Preflight. A thorough preflight inspection would have most likely caught a dead or dying servo.
