I was wondering what is the distribution of the reasons you crashed... Let's make it like a poll:

1) Mechanical failure or radio interference(hey! it's not my fault)
2) Poor maintenance (oops! did I forget to put loctite on that?)
3) Dumb Fingers (left? left?!? I mean right!)

2-once, tail servo was stripped in a prior crash and I failed to find it. My first mandatory auto went well.

1) Never (Spektrum DX7)
2) Once or twice. Not enough loctite for the oil drenched set screw for the tail rotor hub, causing loss of tail control.
3) Mostly. What is muscle memory in normal flight has happened to disappear in a bad situation, when I start thinking "oh no, this is not going well"...
